I confirm that I have been informed about the nature, steps, and purpose of the beauty service(s) I have selected at Luxury Brows & Lashes Studio. I understand that the specific service(s) may include, but are not limited to:

• Eyebrow shaping, lamination, or tinting
• Eyelash lifting, tinting, or extensions
• Other beauty treatments as discussed with my technician 

2. Possible Risks & Side Effects

I understand and agree that, despite the professionalism of the technician, the procedure may have possible side effects, including but not limited to:

• Redness, itching, swelling
• Allergic reactions to products
• Hair breakage or loss if aftercare is not followed
• Unsatisfactory visual result due to individual characteristics 

3. Client Responsibilities

• Inform the technician of any allergies, skin conditions, chronic illnesses, pregnancy, or medications
• Follow aftercare instructions
• Understand that results may vary 

4. Liability Waiver

I acknowledge and accept that Luxury Brows & Lashes Studio, its owners and staff are not responsible for: 

• Allergic reactions or side effects
• Changes in hair/lash condition
• The need for correction or repeat procedure
• Any consequences resulting from failure to follow my responsibilities 

5. Consent

I confirm that I have received full information about the procedure and possible risks, had the opportunity to ask questions, and agree to proceed voluntarily.
